Studio Tuition and Enrollment Policies:

The Studio Calendar:
This is a very important document at the studio. Students will have a personal
appointment schedule created from our studio calendar. All appointments are pre-booked
using our, "Quarterly," system. Please view our 2010 studio calendar

Studio Policies:
For studio polices and enrollment terms please view our student agreement or
call us directly at: 774-2722

Billing, Attendance and Payment Policies:

(a). Billing is done directly from the, "Student Account Statement."

(b). We use 30 day and 3 month "Quarterly," payment plans.
         Class dates are pre-assigned according to our studio calendar.

(c). Full-Time students enroll for the calendar year, (Jan. - Dec.),

(d). All new students are required to take our, "New Student 4-Week Program."

(e). The studio does not offer; make-up lessons, re-scheduling of classes, or credit
         of any kind for missed appointments. If any class cannot be attended, the
         missed appointment will still be billed out to the student with no re-imbursement.

(f). If the teacher is unable to conduct the lesson for any reason, the lesson will be either:
1). Re-scheduled to another day/time.
2). Credited back to the student by refunding the student’s tuition fee.

WEEKLY STUDENTS || Weekly Pre-Booked Private class: 30 min. or 60 min.

Weekly Appointment: 30 min. class: *$25.50 - $27.50

Weekly Appointment: 60 min. class: *$51.00 - $55.00

*Tuition rates decrease when larger tuition packages, (such as the Quarterly
Payment Plan), are purchased.

Note: Tuition fees are subject to change with 30 days notice.

PRELUDE PROGRAM || Bi-Weekly Pre-Booked Private class: 30 min. or 60 min.

Bi-Weekly Appointment: 30 min. class: *$25.50 - $27.50

Bi-Weekly Appointment: 60 min. class: *$51.00 - $55.00

*Tuition rates decrease when larger tuition packages, (such as the Quarterly
Payment Plan), are purchased.

Note: Tuition fees are subject to change with 30 days notice.

ALLEGRO PROGRAM || Advanced Guitar Pre-Booked Private class: 60 min.

4 Appointments: 60 min. classes: $435.00
- Includes (2) Instructors ...Plus... the Creative Guitar Advanced Material Package

Note: Tuition fees are subject to change with 30 days notice.

IMPORTANT:
Please report any errors on your Student Account Statement
to the office immediately. We will be billing your credit card
according to your Personal Account Statement.

We will process your credit card according to your account statement until you issue the studio
a written notice of withdrawal. All processed sales are final. Report errors immediately.

Note:
Once processed, Creative Guitar Studio does not refund fees for any reason.

* Students may stop lessons at any time with 30 days advance notice. A $50.00
withdrawal fee is billed to the student's account if 30 days notice is not given.
